All new commercial members of Miami-Cass REMC must complete a Commercial Membership Application and Agreement Form which is available by clicking here.

All new commercial members must appear in our office and provide a valid identification with photo.  If you are unable to come into the office this form can be emailed, faxed, or sent to us.  The form is available by clicking here.
